Thank you for expressing interest in Cataldo Ambulance Service, Inc. Please download and complete the Employment Application, CORI Request Form and Personal Info form and return them for processing. Be sure to include copies of your EMS certification(s), CPR card, driver’s license and motor vehicle driving record, as this will assist us in processing your application.

Step 1: Complete and Submit Online Application by attaching it to a new email. Click for help attaching a file to your email?

If you have trouble downloading the application--try the one for older computers--you must fill this application out by hand and mail to Cataldo Ambulance.

Step 2: Download and print CORI forms and bring into your interview with Human Resources.

Step 3: Someone from Human Resources will be in touch with you to set up an interview for employment.
